America’s worst nursing homes are designated by federal watchdogs as “one-star” facilities. According to CMS methodology, the bottom 20% of nursing homes within each state receive the lowest “one-star” rating in the health inspection component, based on the number, scope, and severity of deficiencies (citations) found during state health inspections, complaint investigations, and infection control surveys; staffing levels are evaluated separately as a distinct rating component.
