A computer displaying said information was left on a medication cart in a hallway where it was visible by staff, residents, and visitors.

Failed to Ensure a Clean & Safe Environment

Per the Medicare inspection conducted on 10/15/2021, at least ten residents had visible damage to the walls of their rooms. This damage violates residents’ right to a clean, home-like environment. Further inquiry indicated there is no policy regarding upgrades or repairs to residents’ rooms.
