Failure to Ensure a Sanitary, Homelike Environment

According to records from Medicare.gov, in the most recent inspection on 3/30/2022, inspectors found ProMedica (Imperial) failed to ensure residents’ rooms were clean and free of accident hazards. They also failed to ensure residents could call for help when needed. In more than one instance, residents’ call bells were hanging out of their reach behind furniture or on the floor.

Failure to Assist Residents with Personal Care

Per Medicare.gov, during a complaint inspection on 2/25/2022, inspectors found residents at ProMedica (Imperial) who needed assistance bathing were not bathed regularly, and staff was not trained in all aspects of personal care. Assisted personal grooming, such as washing hair, was not routinely performed.

Failure to Provide & Implement an Infection Protection & Control Program

According to Propublica.org, during an inspection on 12/20/2018, inspectors observed an RN changing the resident’s suprapubic catheter drain line and gastronomy tube. The RN failed to follow infection control practices during a dressing change for a resident. The facility also failed to maintain infection control practices for the maintenance of the resident’s bed mattress.
