Clinical records reviews and interviews with staff revealed that the facility failed to keep medication error rates below 5%. Cathedral Villages error rate was at nearly 7%. Inspectors also observed several medication errors while at the facility, on the day of the inspection.
An interview with a resident from the sample population revealed that she was scheduled to receive two showers a week, and wished to receive more, if possible. However, the resident stated that sometimes she only received one shower per week. Records confirmed that she was not receiving proper hygiene-related care.
Grievances from June 12 and 15 in 2021 were not properly investigated. Both grievances referenced a pattern of neglect for residents’ grooming, hygiene, and other activities of daily living. An interview with the resident who submitted the June 15 grievance indicated that the facility was understaffed and the nurses overburdened. This appeared to be the reason for the alleged neglect.
