Inspectors found residents with personal items such as an iPad and an oil painting had gone missing without satisfactory explanation from any staff member, and with no mention of the items on the resident’s inventory report.

Residents’ Needs for New Eyeglasses Went Ignored

Inspectors found residents having had eye exams over a year prior who had not received new prescription eyeglasses. One resident was wearing eyeglasses with scratched lenses and only one ear piece. Another had an eye exam 22 months prior, needed new glasses, and had lost his old glasses in the meantime.

Residents are Subject to Physical & Verbal Abuse

On September 29, 2021, inspectors responded to a report of resident abuse. Staff were seen striking a difficult resident across the face with an open hand and heard using racial slurs.
