Medicare’s most recent inspection of Colonial Park Care Center noted the following conditions:

Inadequate Staffing

In June and March of 2021, the Pennsylvania Department of Health responded to multiple complaints related to staffing issues at Colonial Park. Inadequate staffing resulted in unsanitary conditions at Colonial Park.  Additionally, the inspections revealed multiple instances of inadequate medical care.

Patient Neglect

The Pennsylvania Department of Health found an instance in June of 2021 in which a patient was overmedicated.  Additionally, the Pennsylvania Department of Health found a staff member failed to aid a patient suffering from incontinence.

Risk of Injury

The Pennsylvania Department of Health found that in June of 2021, a physician’s orders regarding medical care were not followed or even placed in residents’ Care Plan, resulting in inadequate care for some incontinent residents, oxygen tanks used by residents not marked with the date first put into use, and vision problems were not addressed.  Additionally, in March of 2021, staff members failed to follow a physician’s order regarding glucose monitoring which resulted in a resident being hospitalized.

Colonial Park Care Center is a 1-Star CMS Rated Nursing Center

The deficiencies in health inspections, staffing, and quality measures earned 1 star of 5 from Medicare in its 2022 review and imposed $10,400 in fines as a result.

Lawsuits & Federal Fines Against Colonial Park Care Center

In addition to the Medicare inspection, the Pennsylvania Department of Health inspected Colonial Park on March 15, 2022 and found non-compliance with the law governing long term care facilities:
  • Insufficient infection prevention and control
  • Failure to make appointments and provide transportation to residents requiring treatment and assistive devices for decreased hearing and vision
