Concordia Village of Tampa Complaints

On at least one occasion a resident fell from a wheelchair unsupervised, fracturing her hip, but was not taken to the hospital until more than 24 hours later. No staff documented the suspected abuse, unwitnessed fall or conducted an internal investigation, as required.
