Medicare’s most recent inspection of Garden Spring Nursing and Rehabilitation Center in January 2022 noted the following conditions:

Jeopardy to Patient Health & Safety

A January 2022 inspection by the Pennsylvania Department of Health found multiple health and safety violations. Residents were subject to restraint with no note on file. Residents with wounds did not receive the ordered wound care. Residents with tracheostomies were not regularly suctioned or cannula secretions regularly cleaned. The doctor recommended dental care for one resident who had broken teeth and exposed roots, and that care had not been provided sixteen months later when the inspection took place.

Failure to Provide Care

In that January 2022 inspection, Garden Spring Nursing and Rehabilitation Center was cited for failing to offer assisted showers for residents who cannot shower themselves, in some cases only offering a shower twice in one month and in one case, offering no shower for a month.

Quality of Life Deficiencies

The most recent Medicare inspection in January 2022 found worn and broken furniture, lack of hot water for residents’ use, and unsanitary conditions in the kitchen and cleaning closets.

In May 2022, Medicare rated Garden Spring Nursing and Rehabilitation Center one star (out of five) based on three categories: health inspections, staffing, and quality measures.
