Inspectors noted residents’ conditions indicating staff at Lakeland Nursing and Rehabilitation Center failed to provide appropriate care for residents who are continent or incontinent of bowel/bladder, failed to provide appropriate catheter care, and failed to provide appropriate care to prevent urinary tract infections.
