Per the Medicare inspection conducted 6/7/2021,Lancaster Nursing and Rehabilitation Center failed to treat a patient with CHF (a condition where the heart does not pump blood as it should, causing increased fluid retention, swelling in arms or legs) for more than two days after a 22.2 lb. weight gain was noted. The Medicare inspection on 6/7/2021 revealed that—despite the lack of written consent—a mitt was placed on a resident’s left hand to prevent him from pulling on his medical tubing. The mitt met criteria as a restraint because it could not easily be removed by the patient, and restricted his freedom of movement.
On February 23, 2021 the resident’s responsible party clearly stated that they did not want the resident to wear a mitt. Clinical records from March 22, 2021 state the resident was still wearing the mitt.

LancasterOnline reported “Two Lancaster Nursing and Rehabilitation Center residents recently ended up in an emergency room due to the facility’s failure to keep them safe, one after nursing home officials failed to act for more than 12 hours after learning he had gone missing from the facility.”
Medicare gave Lancaster Nursing and Rehabilitation Center only one star out of five for health, staffing, and quality measures in its most recent 2021 inspection.
