Medicare’s most recent inspection of Mountain View Nursing and Rehabilitation Center noted the following conditions:

Low Staffing

Mountain View Nursing and Rehabilitation Center’s shortcomings extend to persistent staffing issues, both lack of training and supervision and understaffing. Understaffing causes residents to wait 30 minutes or more for assistance having sounded the call bell.
Insufficient staffing can leave residents vulnerable to injuries from nursing home neglect or abuse.  A 2019 study by the Long Term Care Community Coalition indicated that Mountain View Nursing and Rehabilitation Center was nearly a candidate for intervention by the CMS Special Focus Facility (SFF) Program due to several factors including poor staffing.  Staffing issues continue to persist as found by a July 2021 inspection by the Pennsylvania Department of Health.

Patient Neglect

An honest review by a former employee said “she witnessed drug trading and using among the staff, resulting in the neglect of a resident who should have been supervised while he ate and nearly died from choking on his food.
In July 2021, the Pennsylvania Department of Health found several instances of patient neglect.  The inspection noted that staff failed to respond to patients’ calls for service.  The inspection also found that staff failed to clean the facility which resulted in dirty bathrooms and unsanitary build up of garbage.

Low Medicare Health Inspection Rating

CMS rated Mountain View with one (out of five) stars due to deficiencies such as failing to establish and maintain an effective cleaning schedule of the facility, resulting in unclean conditions such as excessive dirt and dust, vermin in and on the property, old spills and food remaining on the floor, and garbage overflowing and not picked up daily.
