According to the Medicare.gov report, inspectors observed on 7/15/2021 that facility staff failed to meet residents’ personal care and grooming needs. Facility staff failed to bathe residents and assist them with dressing and grooming tasks, specifically trimming nails and toenails. The staff also did not address skin concerns, specifically a rash.

Failed to Prevent or Avoid Medication Errors

The report on Medicare.gov states that during the inspection conducted on 7/15/2021, inspectors found facility staff failed to ensure residents received medication as ordered. A review of a resident’s care plan revealed the resident had not received the correct number of medication doses at the correct times.
