This inspection also revealed unsanitary conditions in the kitchen used to prepare residents’ food. Dust, a black substance, and a white film of some kind had been allowed to build up around the cooking areas. The black substance was also found around the door of their single functioning refrigerator.
Staff failed to follow up with a resident’s urology diagnosis which required a second visit. Physician visits should have been scheduled for new residents every 60 days but were not. Elevators were out of service for long periods of time and not reported as statutorily required. Staff was seen administering medications with their bare hands and not washing their hands, flouting infection spread protocols.
Accessible water was contaminated with Legionella at the time of the most recent inspection on 8/12/2021, and water was not readily available to residents with which to wash their hands or shower independently. Further, residents who cannot shower independently report being showered only once in 30 days, or four weeks. Water restrictions had been implemented on 3/21/2021 and were not fully resolved at the time of inspection, several months later.
Medicare inspectors also found that Parkhouse failed to ensure a safe, functional, sanitary, comfortable home environment for residents infor one of their two shower rooms. Only one of the showerheads in the contaminated area was fitted with a Legionella filter, and it had extremely low water pressure.
The Centers for Medicare and Medicaid Services (CMS) rates Parkhouse 1 star out of 5 as of March 2022, taking into consideration health inspection, quality, staffing, and RN staffing.
Parkhouse has been fined $47,800 to date for its continuing deficiencies in resident care.
