The inspection on 7/16/2021 revealed facility staff failed to maintain a medication error rate below 5%, per standard guidelines. Two of the six residents from the sample population had a medication error rate of 16%.
During the 7/16/2021 inspection, records showed facility staff failed to use two persons—as safety guidelines require—when transferring a resident with a medical lift. This error resulted in the resident slipping from the transfer pad and sustaining multiple serious injuries, including rib fractures and bleeding.
