Based on Medicare inspectors’ observations, interviews with residents and staff, and medical records review, they found that facility staff failed to ensure a resident was protected from physical abuse, and failed to follow policies to report the incident and prevent future abuse.
Per these observations, a resident was roughly handled by a staff member when being assisted into a wheelchair. A second staff member then forcefully pulled back the wheelchair, causing the resident to fall to the floor and sustain an abdominal injury. Both staff members then roughly handled the resident while placing them back into the chair and returning them to their room.
In addition to allegations of abuse, the 6/8/2020 Medicare inspection also found these incidents were not being appropriately reported. Though an incident in which a resident was shoved out of their wheelchair and sustained an abdominal injury was recorded, there was no documentation to indicate the Nursing Supervisor or Administration was notified. The facility’s regional coordinator also stated she was unaware of any abuse, and stated that reports of such should have started an investigation into the matter. There was no indication of an investigation.
