Based on a clinical record review conducted by Medicare inspectors on 8/5/2021, the facility failed to follow physicians’ orders for three patients in the sample population. One resident’s tube feedings were not correctly documented. The facility failed to obtain a physician’s order for hospice care services for a second patient, and a third was administered insulin when it should have been withheld.
The inspection on 8/5/2021 also revealed facility staff failed to properly assist residents with personal hygiene and grooming tasks. Some residents require help with bathing, hair, and nail care. Several residents were not receiving showers according to their care plan, while others were not receiving assistance to keep their nails trimmed and clean.
After their most recent Medicare inspection, conducted on 8/5/2021, inspectors rated Yorkview one out of five stars for health and two out of five for staffing and quality measures.
